A physician's referral may be required by some insurance carriers and worker's comp.
Please remember to have this paper with you before your first appointment, if required, or you cannot be treated. If you do not have a referring physician, we can assist you, just call.

Prices:
Prices depend on type & length of treatment. Initial evaluation & consult may incur a separate fee.
There are separate NADA programs for Smoking Cessation & Weight Loss & Addictions.
On site event treatments are dependent on mileage radius, length & type of treatment.
Any other treatment/pricing: per agreement

**treatments may vary in length depending on treatment and patient condition.
**missed appointments without notice & cancellations with less than 24 hours notice will be charged a $25 fee.

Credit or Debit payments are accepted.
**A8 accepts participating insurance, worker's comp, and personal injury but is not "in-network" with any insurance carrier.
